Cancellations and Rescheduling

Late Arrivals

Insurance

Outstanding Balances

Outstanding balances must be paid within 30 days of billing. Failure to pay may result in the suspension of services and referral to a collection agency. You will be responsible for all collection costs, including reasonable attorney fees.
